VASD - 5-11 Year Old Vaccination - Second Dose

As part of our district commitment to support COVID-19 vaccinations for all eligible individuals, we hosted three clinics in November where we were able to vaccinate over 1,050 of our 5 to 11-year-old students.  In order to be considered fully immunized, it is recommended that each child receive a second dose of the Pfizer COVID-19 vaccine three weeks after the first dose.

This weekend the VASD, in partnership with Mount Horeb Pharmacy, will be hosting second dose vaccination clinics for any 5 to 11-year-olds who have received the first dose of the Pfizer vaccine at least three weeks ago.  The clinics are open to any students in that age range who are eligible.  The clinics are also open for first doses as well.  If your child has turned 12 years old since the first dose, please let us know when you arrive so the proper dose is delivered.

The clinics will be open during the same times as they were three weeks ago, but the location has changed.  We encourage you to come at approximately the same time as you came last time if that works for you. Otherwise, please come at any time during the clinic hours.  If you are unable to make these times, we will be hosting another clinic next week on Wednesday, December 8th from 3:30-7:30 pm at Savanna Oaks Middle School, or you may look up other vaccination locations on the Public Health Madison Dane County (PHMDC) website.

The clinic hours and location this weekend are:

December 4th from 10:00-3:00 at Sugar Creek Elementary School

December 5th from 11:00-4:00 at Sugar Creek Elementary School

Sugar Creek Elementary School is located at 740 N Main Street, Verona.  Please park in front of the school and enter through the main entrance.  There will be a banner outside the entrance.

Prior to your arrival at the clinic, it is important to complete the consent form.  Please go to the link below to complete the online form:

Saturday 12/4: Consent Form for 12/4

Sunday 12/5: Consent Form for 12/5

In preparation for the clinics, please be aware of the following:

  • Parents should plan to accompany their child(ren) to the appointment.
  • For second doses, you MUST bring your vaccination card.
  • You will need to wait 15 minutes after the vaccine to ensure there are no side effects.
  • Your child(ren) should eat and drink before their vaccination appointment.
  • Your child may bring a comfort toy or item if that helps them feel comfortable.
