Special Education and 504 Record Requests
Requesting Special Education or 504 Records
Verona Area School District (VASD) manages Special Education and 504 records. Please follow the instructions below based on your relationship to the student.
1. School Districts & Colleges

K-12 School Districts: Submit written transfer requests to Rachelle McCullough or via fax at (844) 787-4713.

Colleges/Universities: Please send a Release of Information (ROI) form signed by the former student to Rachelle McCullough or via fax at (844) 787-4713.
2. Parents & Guardians (Students under 18)
Parents or guardians of current or former students under age 18 may submit a written request to Rachelle McCullough. Your request must include:

The student's full name.

The specific delivery method (email address, mailing address, or fax number).
Important: Record Retention Policy
VASD is only required to maintain Special Education and 504 records for one year after a student graduates or leaves the district. * When a student transfers, records are forwarded to the new school.
3. Students & Guardians (Students 18+)
The rights held by a parent regarding his or her minor child transfer to the child when the child reaches the age of 18 years. However, the parents of an adult student who continues to be financially dependent upon the parents may continue to have access to their child’s pupil records, unless the adult student notifies the school in writing this information may not be shared with the parents without the student’s permission, Wis. Stat. sec. 118.125(2)(k).

Former Students (18+): Complete the VASD Former Student: Special Education/504 Record Request Form. You must provide proof of identity.
- Parents/Guardians of Former Students (18+):
- If you are a parent/guardian of a former VASD student over the age of 18 and the former student is either financially dependent upon you or you are their court appointed guardian - Complete the VASD Former Student: Special Education/504 Record Request Form.
- Former students over 18 who are not financially dependent upon their parents or do not have a court-appointed guardian must request the records themselves. The former student must complete/submit the VASD Former Student: Special Education/504 Record Request Form and provide proof of identity. The adult student has the opportunity within the body of the form to indicate to whom they wish the records be sent.

4. General Education Records (Transcripts, Grades, etc.)
For cumulative records such as transcripts, immunization records, attendance, or disciplinary history, please contact the last school the student attended within the district directly.
